» Physician Bio:
• Michael A. Cromer M.D.
Family Practice
Dr. Cromer is board certified by the American Academy of Family Practice and by the American Board of Addiction Medicine. Dr. Cromer has been practicing since 1989.

He attended medical school at the University of Oklahoma in Tulsa, OK. He completed his family practice residency at Bayfront Medical Center in St. Petersburg, FL. He started Northwest Family medical Center as a solo practice in 1989. He is also Medical Director of the Addiction Recovery Unit at Town & Country Hospital and Passport Health both located in Tampa, FL. Dr. Cromer is a member of the Christian Medical and Dental Association, American Academy of Family Physicians, Florida Academy of Family Physicians, and American Society of Addiction Medicine. He is married to Carol and has two grown sons and one daughter.

• Heidi Nagel, MD
Dr. Nagel graduated from the University of Florida in 1997 with a Bachelor of Arts Degree in Music with an emphasis on Computer Science. After graduation, she worked as a computer programmer for a medical laboratory, Nortel, and Nintendo. She then received a Bachelor of Science Degree in Biomedical Science from the University of South Florida in 2004. Dr. Nagel attended medical school at the University of Florida graduating with multiple awards in community service and research from the Institute Pasteur in Paris, France. She did her residency training in Pediatrics at Wake Forest in Winston-Salem, NC, then transitioned into Family Medicine at the Medical University of South Carolina in Charleston, SC, and Family Medicine at Latrobe Area Hospital in Latrobe, PA in 2014. She is a member of the American Academy of Family Physicians and Florida Academy of Family Physicians. Heidi enjoys playing her violin, listening to classical, Irish, and bluegrass music, hiking, traveling, and reading.

• Kelly Dunn, PA-C
Physician Extender

Kelly Schrank Dunn graduated from the University of Illinois Urbana-Champaign in 1998 with a Bachelor’s degree in cell and structural biology. After working in a pediatric office for 2 years, she went back to school to further her working career. She graduated from The University of Florida in 2002 with a Master’s in Physician Assistant Studies and has been working in family practice since graduation. Kelly has been with Northwest Family Medical Center since 2004 as a Physician Assistant. she is a member of the American Academy of Physician Assistants and the Florida Academy of Physician Assistants.

• Christian Neller, PA-C
Christian Neller, PA-C, graduated from the University of South Florida in Tampa with a Bachelor's of Science in Biomedical Sciences and Minor in Public Health. She then went on to complete her Master's degree in Clinical Medical Science at Barry University. She is certified by the National Commission on Certification of Physician Assistants. She joined Northwest Family Medical Center in January, 2011. She is a member of American Academy of Physician Assistants and the Florida Academy of Physician Assistants. She also works in the field of addicition medicine at the Town & Country Hospital Addicition Recovery Unit.

• Ashley Collum, ARNP
Ashley Collum graduated from the University of South Florida in Tampa with a Bachelor’s degree in Nursing. She then attended the University of South Florida’s nurse practitioner program to become a family nurse practitioner. She is certified by the American Nurses Credentialing Center. She is a member of the American Nurses Association, West Coast Council of Advanced Nursing Practice, and the American Association of Nurse Practitioners. She enjoys spending time with her family and friends, traveling, and reading.

• Megan Wagner, PA-C
Megan Wagner graduated from Southeastern University in Lakeland, Florida with her bachelors degree in Biology and with minors in Chemistry and Religion. She then attended Barry University’s Physician Assistant Program and holds her Masters degree in Clinical and Medical Sciences.
She is board certified by the National Certifying Commission of Physician Assistants, is a member of the American Academy of Physician Assistants as well as the Florida Academy of Physician Assistants.
She is also skilled in cosmetic procedures such as Botox, facial fillers, skin rejuvenation and Sclerotherapy.

» Procedures & Services Provided:
Family physicians are trained to deliver a broad range of care to the entire family. Family practice emphasizes preventative and continuous care from newborns to the geriatric population.

In an effort to provide quality care for our patients, we are equipped to perform the most common diagnostic procedures in our office including lab work, x-ray, EKG, and bone density testing. Due to our special training, we are able to perform infant circumcisions, cardiac stress tests, flexible sigmoidoscopy, colposcopies, and newborn care. Our services include, but are not limited to, routine care of newborns, children, and adults, immunizations, and school/sports physicals. When indicated, we treat a variety of sports and musculoskeletal injuries, lacerations, and perform minor skin surgeries. If your condition should warrant services that cannot be provided in our office, the appropriate referral will be made.

MISSION STATEMENT


To provide the most compassionate and best quality care possible for each individual in order to enhance his or her quality of life. This includes not only physical care but also spiritual, mental, and educational support.


PHILOSOPHY


We believe that medical care is best delivered when the knowledge of medicine is mixed with the art of compassion. We want everyone who visits us to feel a difference in our office because of what we stand for. From the person who greets you to the person at check out, we want you to feel that you matter to us. You are more than just a number, you are a person to whom we truly want to give good service and care.

In carrying out our philosophy, we have become one of the largest practices in Hillsborough County. As such, there is no way that one or two doctors can handle our daily influx of patients. We use physician extenders, both nurse practitioners and physician’s assistants, in delivering medical care to our patients. You can always ask for your provider of choice, but we ask that you please be willing to be seen by any available provider. This way you will be seen in a more timely manner. We confer about patients on a daily basis. A physician oversees every visit seen by a physician extender and most of the time is immediately available to step in for a consult, give further information, or lend a hand.

Cash, Check, and Credit card (Visa and MasterCard, both debit and credit). Payment is due at the time of visit.

» We accept the following insurances:
We accept all major forms of insurance. Unfortunately, we are not accepting Medicaid, Humana Gold choice HMO, PHCS, or Tricare as a primary insurance.

» Additional Information
NON-COVERED SERVICES

Please be aware that certain conditions and visits are not covered by health insurance.
These include, but are not limited to:

- Weight Loss Assistance with Medifast, Phentermine, or other diet aids.

- Drug and Alcohol Detox

- Laboratory blood draw fees (on some plans)

- Preventative health physicals and some procedures or blood work (ex: flu shots, other immunizations, PSA, screening EKG’s, Chest X-ray, or Flex Sig) are not covered benefits on some insurance plans.

- Cosmetic procedures

The patient is responsible for non-covered services even when it is bundled with a covered office visit.
